336 S West End Ave, Lancaster, PA 17603Hamilton Arms Center is currently looking for a Part Time Monday-Friday Registered Nurse Assessment Coordinator (RNAC) with flexible scheduling to assist with joining our growing MDS team! The schedule would be 24 hours a week, with 3 full 8 hour days.

On site 3 days a week, full 8 hour days for 24 hours a week- remote work will not be consideredRegistered Nurse (RN) with effective leadership & assessment skills in a long-term care settingMust have knowledge of state and federal regulationsMust have, as a minimum, two (2) years of experience as an MDS CoordinatorMust have knowledge in rehabilitative and restorative nursing practices in the long term care setting.
Proficient in PA CMIRegistered Nurse Assessment Coordinator (RNAC) must have Medicare billing knowledge and experience.
Knowledge of PPS (PDPM)Experienced in Managed Care and concurrent reviews.
Must successfully complete a post offer, pre-employment criminal background check and physical / drug screen.
Responsibilities of the Registered Nurse Assessment Coordinator (RNAC):
Conduct and coordinate the completion of the resident assessment in accordance with the requirements of this state and policies and goals of the Facility.
Conduct and coordinate the completion of the RAI process in accordance with current rules, regulations, and guidelines that govern the resident assessment in accordance with Medicare, Medicaid, OBRA, and other payer programs.
Develop, implement, and maintain an ongoing quality assurance program for the resident assessment.
Participate in Facility surveys (inspections) made by authorized government agencies.
Monitor the Facility's QI and QM reports to ensure that appropriate corrective action can be implemented when potential problem areas occur.
Provide initial and ongoing education with the interdisciplinary team involved in the RAI process.
